Family Support Services encompasses a wide range of programs and support services available to families and individuals with developmental disabilities living in Saratoga County.
The following are some of the many programs and services offered:
At Home Services: At home services include several programs that support families who have their family member living at home. The individual must be enrolled in the Medicaid Waiver Program and be deemed eligible for services by OPWDD to be considered for this service. Services are often dependent upon availability of service hours. Home visits may be scheduled to determine interest and to identify goals, and an Individual Support Aide will be assigned to work with the individual and family to carry out services identified in the plan. Referrals must come directly from the Service Coordinator to the Assistant Director of Family Support Services. Examples of these services include:
Community Habilitation: A program designed to encourage community integration utilizing existing local resources for the purpose of skill building based on goals in an Individual Service Plan, or ISP.
Respite: Relief program for parents and families who are responsible for the primary care of people with developmental disabilities.
Family Reimbursement: A cash subsidy program available on a limited basis that assists families with expenses incurred while caring for an individual living at home. Through an application and committee review process, families are able to purchase services and goods based on the individual’s and family’s needs.
Self-Directed Services: Self-Direction gives individuals the chance to choose their own services so they can live the lives they want. Through self-direction, individuals will have more flexibility to choose the right supports, including the staff they would like to work with and a schedule that works best for them. They will get to choose the services they want and need.
Autism Support: Various services dedicated to individuals on the Autism spectrum and their families, such as Adult Social Clubs, Caregiver Support Groups, and much more!
